Just click on top icon - Family Account - Return User Login. All accounts transfer over from the middle school to the high school. **If you are a new student to the district or participating in a sport for the first time, please contact your school nurse via email (see emails below) prior to registering in order to have access to the registration process** PHOTOCOPIES/SCANNED/FAXED PAPERWORK WILL NOT BE ACCEPTED!!!!! Physical/Health History Update forms MUST BE ORIGINALS. Please turn all forms in to the main office or the nurse’s office. Physical Examinations Students registering for Athletics must have a current physical examination on file in the nurse’s office before the student is authorized to participate in practice/tryouts . To ensure clearance for participation, physicals should be completed and turned in prior to the dates mentioned below. Physicals are valid for 1 year from date of the last exam. Download your NEW Sports Physical Form HERE . If the athlete has a current physical on file in the nurse’s office , you DO NOT need a new physical. Please submit the Health History Update Questionnaire i nstead. An original copy must be submitted to your school nurse. Download the form HERE . If the athlete has Asthma and uses an inhaler please click HERE . If the athlete has an allergy that requires the use of an Epipen please click HERE. Attention High School Athlete’s ONLY : If you are going to be participating in any OUT OF SEASON workouts, you MUST complete the corresponding season form and return to Mrs. Redler in the nurse’s office prior to participating.
